If you have been selected for a civil service position with the U.S. Army Research Laboratory (ARL) or a tenant activity, you will need to in-process. In order to process your appointment, you will need to complete in-processing forms. Please click on the In-Processing Forms link below. Print all applicable forms in hard-copy format. Complete and sign each form. You will receive a Welcome Letter with instructions as to where and to whom specifically send your completed forms.

General Information

Forms

I-9 - Employment Eligibility Verification. Applicable for all new employee in-processing. Your Directorate Administrative Officer and/or Supervisor must complete this form on your first day of work. Acceptable documents are listed on the back of the form, and detailed below under "Special Instructions." Please read and be sure to bring the needed form of ID listed on the form.

SF-144 - Statement of Prior Federal Service. Applicable for all new employee in-processing. This information is voluntary; however, failure to do so may result in you not receiving credit for prior Federal service. Answer all questions on page 2, sign, and date.

SF-181 - Race and National Origin. Applicable for all new employee in-processing. This information is voluntary. Your failure to do so will have no effect on you or on your Federal employment. This information will be used in planning and monitoring equal employment opportunity programs. Complete the top of the form and place an "X" in the box next to the appropriate category.

SF-256 - Self-Identification for Handicap. Applicable for all new employee in-processing. This information is voluntary. Your failure to do so will have no effect on you or on your Federal employment. The information you provide will be used for statistical purposes only and will not in any way affect you individually. Complete the top portion of the form and enter the appropriate code (code is identified in bold numbers).

OF-306 - Declaration for Federal Employment. Applicable for all new employee in-processing. Answer all questions, sign, and date in block 16A. Read the Privacy Act Information.

SF-1199 - Direct Deposit Form. Applicable for all new employee in-processing. Failure to complete this form will cause a delay in your payroll check. Employee should complete and sign. (Note: C. Claim or Payroll ID Number is your SSN). Take this to your bank or financial institution and have them complete Section 3. Return to the CPAC.

Varies - State Tax Withholding Form. Applicable for all new employee in-processing. The State tax form should be for the state of your permanent residence. Employee should complete and sign.

W-4 - Federal Tax Withholding Form. Applicable for all new employee in-processing. Employee should complete and sign.

ARL-213 - Education, Professional License or Certification and Foreign Language Skills Questionnaire. Applicable for all new employee in-processing. Information is voluntary, but failure to do so may delay the processing of education and training related actions or result in missed training opportunities. Answer all questions that apply, sign, and date form on page 6.

N/A - Selective Service Registration. Applicable for all new employee in-processing. Answer all questions, sign, and date. If you are retired from military service, please forward a copy of your DD 214 (discharge papers).

SF-15 - Application for 10-point Veteran Preference. Applicable for all new employee in-processing. Complete and sign form. All new employees who are veterans and claiming a thirty (30)-percent compensable disability (i.e., ten (10) point preference eligible).

SF-813 - Verification of Military Retiree's Service in Non-Wartime Campaigns or Expeditions. Applicable for all new federal employees who are retired military. Answer all questions, sign, and date. Specific instructions are located at page 2.

N/A - Employee Military and Retirement Identification. Applicable for all new employee in-processing. Employee should complete. If not retired military, please complete the top portion and annotate "not applicable" as appropriate.

SF-86 - Questionnaire for National Security Positions. Applicable for all new employee in-processing requiring a security clearance/investigation. Employee should complete and sign. SF-86 instructions can be found here and SF-86 Policy Implementation can be found here.

SF-61 - Appointment Affidavit. Applicable for all new employee in-processing. Employee should complete the top portion of the form and bring it with them on their first day of employment. You will be given the Oath of Office, and then you will sign the form.

Benefit Information

Army Benefits Center (ABC-C) - The Army Benefits Center (ABC) provides automated benefits support to Army employees through the Employee Benefit Information System (EBIS), the Interactive Voice Response System (IVRS), and trained counselors. The ABC may be accessed at https://www.abc.army.mil. You can contact us by Mail: 303 Marshall Ave., Ft. Federal Employee Health Benefits (FEHB) - Applicable all new employees. As a new employee, you have 60 days from the effective date of your appointment in which to complete an election to enroll in a plan or not to enroll. You must access the ABC, EBIS, or IVRS system to enroll. Health coverage is NOT automatic. The FEHB handbook can be found here.

Federal Employees Group Life Insurance Program (FEGLI) - Applicable all new employees. Basic life insurance coverage is effective on the first day you enter on duty. You have 31 days from the date of your appointment to elect Optional insurance. The FEGLI handbook can be found here.

SF-2823 - FEGLI Designation of Beneficiary. Applicable for all new Federal employees. You do not have to complete this form; however, if you would like to designate a beneficiary, please complete and return to your ARL CPAC Specialist.

Thrift Savings Plan (TSP) - Applicable all new employees/reassignment or transfer employees who have not previously elected participation in TSP.

Thrift Savings Plan (TSP) - TSP 3 Designation of Beneficiary. Applicable for all new employees/reassignment/transfer employees who have not previously completed. You do not have to complete this form; however, if you would like to designate a beneficiary, please complete and return directly to the TSP address listed on the form.

Thrift Savings Plan (TSP) - TSP 19 Change in Procedure Effective 12 June 2006. Applicable for employees in the following categories: (1) Transfers from other agencies outside of Army, (2) Transfers within Army when the payroll office has changed, (3) New appointments when the employee does not have a break in service (4 days or more) from their last federal appointment, (4) Accessions where the employee's break in service is between 4-30 days and the new appointment permits retirement system coverage.

Federal Employees' Retirement System (FERS) - Applicable for all new federal employees and reassignment/transfer employees who have not previously completed.

SF-3102 - Designation of Beneficiary FERS. Applicable for all new federal employees and reassignment/transfer employees who have not previously completed. You do not have to complete this form; however, if you would like to designate a beneficiary, please complete and return directly to your CPAC Specialist.

SF-1152 - Unpaid Compensation of Deceased Civilian Employee. Applicable for all new federal employees and reassignment/transfer employees who have not previously completed. You do not have to complete this form; however, if you would like to designate a beneficiary, please complete and return directly to your CPAC Specialist.
